To streamline communication and coordination, we utilise a Key Worker Model, designating a single point of contact for families. This ensures consistent support, simplifies decision-making, and keeps families involved in every step. The key worker listens closely to your needs, works with the team to develop a comprehensive plan, and connects you with additional experts and community resources when needed.
